Abuse Prevention Requirement Guide

The Fellowship of Christian Assemblies (FCA) is committed to fostering a safe and secure environment for all members, ministries and communities we serve. As part of this commitment, we have implemented an Abuse Prevention Policy requiring all individual members to submit a Vulnerable Sector Check (or alternate check) to maintain active membership.
Please complete the following step-by-step guide to determine the what kind of check you are required to submit. If you already have the correct documentation you will be prompted to upload a digital copy of your document(s). If you need to obtain any documentation, the guide will indicate the which you need to obtain and upload. The requirements differ based on the sector worked in, age, and what check has previously been completed.
Complete the form and click submit at the end to receive the email confirmation. Your confirmation will include the details of which check you are required to submit.
Please only upload documents for one individual. To submit documents for another minister, please complete a new form.
